Linking to Custom Forms

Simple Link

Any Custom Form can be accessed by using a link in the following format:

https://<your_domain>/portal/get_form.aspx?id=<form_guid>
  • <your_domain> = your church's base website (e.g., www.mychurch.com)
  • <form_guid> = This GUID is created when you initially save the form record. To get this value, go to MinistryPlatform and open a Form record and look for the Form GUID field.

Auto Login Custom Form Link

The following link automatically logs in a specific User and views a Custom Form.

https://<your_domain>/portal/login.aspx?dg=<domain-guid>&cg=<contact-guid>&ReturnUrl=<encoded-return-url>

The return url must be html encoded, which you can do using the convenient form below:

Domain  
This is the domain of your portal, starting with 'https://' and ending with '/portal/'
Example: https://demo.ministryplatform.net/portal/
Domain Guid  
For snippets, you can use the [Domain_GUID] token. Otherwise, found in the MinistryPlatform System Domain record. You can find this in the Setup area on the Domains/Accounts page.
Contact Guid  
For snippets, you can use the [Contact_GUID] token. Otherwise, found in the MinistryPlatform Contact record.
Form Guid  
Found in the MinistryPlatform Form record.
Template Name  
Optional: Name of Portal Skin to use. Leave blank for default
Template may not be working at this time

It is currently recommended to generate the URL and link it from an existing web page on your church's site.
